1) Scope
This Policy contains how we collect, use, disclose, and protect information when you visit czoptimization.com, book services, purchase plans, subscribe, or communicate with us (collectively, the “Services”).
2) Important note about coaching (not medical care)
CZ Optimization provides educational and informational health and wellness coaching only. We are not a medical provider, and we do not provide medical advice or create a doctor-patient relationship. (See Terms & Conditions for additional disclaimers.)
3) Information we collect
We may collect the following categories of information:
A. Information you provide

Account / booking details: name, email, phone number, billing address, booking details, and communications.

Payment information: payment status and transaction details. (Payment card details are typically processed by our payment processor and/or Wix and are not stored by us in full.)

Coaching intake information: goals, lifestyle information, and other information you choose to share for coaching purposes (which may include wellness/fitness information).

We retain information as long as reasonably necessary for the purposes described in this policy (including to provide Services, meet legal/accounting requirements, resolve disputes, and enforce agreements). 9) Nevada privacy rights (NRS 603A – “sale” opt-out)
Nevada law provides Nevada consumers certain rights related to the “sale” of covered information and requires operators to provide a way to submit a verified request to opt out of such “sales,” with timing requirements for responses.

What to include: your name, Nevada residency statement, email, and enough information for us to reasonably verify your request.
Our response timing: We will respond within the timeframe required by Nevada law (generally within 60 days, with a possible extension where allowed).
10) Nevada Consumer Health Data Privacy Notice (NRS 603A.400–603A.550)
Because we may collect wellness and health-related information in the course of coaching, Nevada law may treat some information as “consumer health data” (personally identifiable info linked to health status).
A) Categories of consumer health data we may collect
Depending on what you share and what you request, we may collect:

Fitness and wellness goals, routines, and progress notes

Nutrition and lifestyle information you provide

Self-reported symptoms, sleep/recovery information, and other health-status related information you choose to share

Files you upload or provide (e.g., wellness logs, lab reports)
B) How we use consumer health data
We use this information only to provide the coaching services you request, communicate with you, and maintain service records consistent with this policy. Nevada law requires a clear policy and limits collection/use to disclosed purposes.
C) Consent to collect and share
Nevada law generally requires affirmative, voluntary consent to collect consumer health data unless collection is necessary to provide a requested product/service, and requires separate consent to share consumer health data (with limited exceptions).
We do not share consumer health data except as needed to provide Services (e.g., platform tools you choose to use) or with your consent, or as required/authorized by law.
D) Your Nevada consumer health data rights & requests
Nevada law provides rights to request certain actions regarding consumer health data (including access/confirmation and deletion in many cases) and requires a secure means to submit requests.
To submit a request, email info@czoptimization.com with subject: “NV Consumer Health Data Request” and include your name, email, what you’re requesting, and information to verify identity. We will respond as required by law.
11) California Privacy Rights
Residents of the State of California, under certain provisions of the California Civil Code, have the right to request from companies conducting business in California a list of all third parties to which the company has disclosed certain personally identifiable information as defined under California law during the preceding year for third party direct marketing purposes. You are limited to one request per calendar year. In your request, please attest to the fact that you are a California resident and provide a current California address for our response. You may request the information in writing at the address located above.
12) Third Party Add Server Networks
The Website may use third parties such as network advertisers to serve advertisements on the Website and may use traffic measurement services to analyze traffic on the Website. Network advertisers are third parties that display advertisements based on your visits to the Website and other Websites you have visited. Third-party ad serving enables us to target advertisements to you for products and services in which you might be interested. The Website's third-party ad network providers, the advertisers, the sponsors and/or traffic measurement services may themselves set and access their own cookies on your computer if you choose to have cookies enabled in your browser and track certain behavioral information regarding users of your computer via a Device Identifier. These third-party cookies are set to, among other things: (a) help deliver advertisements to you that you might be interested in; (b) prevent you from seeing the same advertisements too many times; and (c) understand the usefulness to you of the advertisements that have been delivered to you. 13) Sole Statement
This Privacy Policy as posted on this Website is the sole statement of our privacy policy with respect to this Website, and no summary, modification, restatement or other version thereof, or other privacy statement or policy, in any form, is valid unless we post a new or revised policy to the Website.
14) Children’s privacy
Our Services are intended for adults. We do not knowingly collect personal information from children under 13 and you must be 18 years or older to utilize our services.
15) Changes to this Privacy Policy
We may update this Privacy Policy from time to time. The “Effective date” above indicates when it was last revised.
